چکیده

While metal-on-metal (MOM) articulations have numerous advantages, recent literature has shown they are not without risk. As use increased in the past two decades, reports began emerging detailing novel complications. The most prominent is aseptic lymphocyte dominated vacuities-associated lesion (ALVAL). [1] Authors have also reported complications from metal hypersensitivity and concerns persist about the long-term effects of metal ions circulating in the body [1]. Painful metalon-metal hips also created the new challenge of distinguishing between metallosis and infection.

Failure of a metal-on-metal total hip arthroplasty from progressive osteolysis.

Ultra-high-molecular weight polyethylene (UHMWPE) wear, debris-induced osteolysis is a frequent cause of failure of total hip arthroplasty. Metal-on-metal total hip arthroplasty eliminates the generation of UHMWPE particulate debris. Although the volumetric wear of a metal-on-metal articulation may be lower than a metal-UHMWPE articulation, the number of particles may be higher. Osteolysis can ...

Unusual presentation of failed metal-on-metal total hip arthroplasty with features of neoplastic process

Metal-on-metal (MoM) total hip arthroplasty (THA) is associated with increased incidence of failure from metallosis, adverse tissue reactions, and the formation of pseudotumors. This case highlights a 53-year-old female with an enlarging painful thigh mass 12 years status post MoM THA. Radiographs and advanced imaging revealed an atypical mass with cortical bone destruction and spiculation, con...

Growth of an intrapelvic pseudotumor associated with a metal-on-metal total hip arthroplasty after revision arthroplasty causing a femoral nerve neuropathy

The development of pseudotumors is not uncommon with metal-on-metal total hip arthroplasty. Pseudotumors that dissect into the retroperitoneal space can cause symptoms of nerve compression. We describe a case of a 53-year-old male with a metal-on-metal total hip arthroplasty who developed mild symptoms of a femoral nerve neuropathy 6 years postoperatively. Revision arthroplasty to a ceramic-on-...

Taper Failure After Large-Diameter Metal-on-Metal Total Hip Arthroplasty.

The use of metal-on-metal bearing surfaces for total hip arthroplasty increased dramatically in the past decade for a variety of reasons, including the advantage of using larger-diameter femoral heads to reduce instability. However, recent research suggesting negative outcomes and high failure rates associated with some of these implants has decreased their use. Further, the use of larger-diame...

Why large-head metal-on-metal hip replacements are painful: the anatomical basis of psoas impingement on the femoral head-neck junction.

Large-head metal-on-metal total hip replacement has a failure rate of almost 8% at five years, three times the revision rate of conventional hip replacement. Unexplained pain remains a feature of this type of arthroplasty. All designs of the femoral component of large-head metal-on-metal total hip replacements share a unique characteristic: a subtended angle of 120° defining the proportion of a...
